Fighting arthrtitis: cartilage lesions repair can happen as a biological and natural process?
Since 6-7 years, the discovery of the availability of mesenchymal stem cells (undifferentiated cells) in the subcutaneous fat of adult and old patients has boosted the research in the mechanisms of biological repair of articular cartilage lesions of different grades of severity.
This discovery – mabe by Chase Western University labs in Cleveland, Ohio, USA, directed by Prof. Arnold Caplan – has pushed the blooming of new therapeutic strategies prompted to the exploiting of this pivotal principles. Arthritis, when it’s primary or a consequence of injuries or diseases, disrupt articualr functionality, destorys cartilage surfaces, trigger a whole series of abnomalities, which lead to sever loss of joint movement.
Until today (even now), there is a wide use of prostethic implants, which, in most of the cases, can recover the joint functionality with good or even total pain relief! But it’s a complex surgery, not always tollerated by patients, with the possibility of severe complications! For this reason, there is absolute need of a new therapeutic strategy, not only for ameliorating pain symptoms, like hyaluronic acid effect, which if a fashionable therapy, but with limited results.
In my opinion, there is need to focus on the prevetion of arthritis in the joint affected by degeneration process, to exploit all the regenerative potential of mesenchymal stem cells of the connective tissues. Normally, as Caplan told me, free intra-articular fat does not exist, so a proper injection of this product can immediately ameliorate joint functionality providing pain relief.
After 9-10 months, it is evident the joint modifications triggered by regenerative process guided by mesenchymal stem cells. In my experience the best feedbacks come from hip, knee, shoulder, , and ankle elbowjoints, but also for hallux rigidus and trapezio-metacarpal arthritis.
A small liposuction, made in local anesthesia with a mild sedation, is enough to harvest the needed adipose tissue with a dedicated cannula from the periumbilical and abdominal area with minimal (2mm) incisions. The harvested tissue is mechanical filtered and washed with a dedicated device to recover the purified injecatable product! With the help of an expert sonographer the injection can be very preceise. After half an hour, the patient can go home with the full support of his own just-treated joints!
